Contact Your Provider

To contact your provider, first decide whether this problem or issue is emergency. If you are experiencing an emergency, call 911 and ask to be taken to an emergency room for evaluation. If you are not experiencing an emergency, please follow the following suggestions:

During Business Hours

Our office is open Monday - Friday, from 8:00 AM to 5:00 PM. To contact your provider during office hours simply call our main office number at (845) 727-7733, option # 6.

After Hours and Holidays

Generally, your routine medical questions and prescription renewals should be handled during regular business hours. If you have a question of an emergent nature after hours, we can be reached by calling our office at (845) 727-7733. Your phone call will be directed to our answering service that will request your name and phone number. The provider on call will be notified of your message and return your call as soon as possible.
