Request a Referral

For a new referral request, please make an appointment with your provider and discuss your request with him or her. Please note that you need to be seen by your provider yearly in order to request a referral.

For an update on your referral, please call our referral coordinator at
(845) 727-7733 option # 4.

**Please allow 3-5 business days for your referral to be completed.

** As a means to expedite your referral request , if you know of a specialist that you would like to go to, please call your insurance company and make sure that the physician is contracted with your insurance company, then advise the referral coordinator of this.

Should an emergency referral need arise, please call our office and ask to speak to our referral coordinator.
