Request a Prescription Renewal

Most prescription renewal requests are done at the time of your visit. However, if you find that your medication is running low, please call our office during office hours at (845) 727-7733 option # 3 and leave the following information:

Your name, phone number, date of birth, medication name and dosage that you are currently taking and name of the pharmacy that you would like us to call your prescription into. Once your provider approves the prescription, your prescription request will be electronically sent to the pharmacy of your choice.

Please do not request medication on our prescription line for a new onset sickness. Our providers will not refill antibiotics without an office visit to assess your condition. Please call our office to schedule an appointment to be seen by one of our providers.

Note: In order to provide optimum care, remember that if your prescription is running low, please do not wait until the weekend or until your medication has completely run out. If you are planning to travel out of town, you should also make sure sufficient supply of medication is available for use on your trip.
